Safeguarding
Concern Form

For concerns about an adult at risk or a child (anyone under 18)

If someone is in immediate danger, call 999.
Do not delay.

Tell the Safeguarding Lead (SL) verbally as soon as possible, then complete this form on the same day. Do not wait to gather every detail, and do not investigate or ask leading questions. Do not promise confidentiality. Record what was said or seen in the person’s own words, and keep any original notes.

Complete the form which will be sent to the SL, Beverley Jacobson.

If the SL is unavailable, or the concern is about the SL: Please contact Christine Brentani, Trustee (Governance and Compliance), Christine.Brentani@employable.org.uk.

If the concern is about the SL and no trustee is available, or if you think a concern has not been acted on, you can contact the local authority safeguarding team (adults or children’s social care), the Police or, for a child, the NSPCC Helpline (0808 800 5000) directly. You do not need permission.
